【ロンドン2017年5月7日PR Newswire=共同通信JBN】Norgine B.V.は7日、第3相DAYB、MORA研究の新たな事後解析を発表し、低容量の1リットルPEGとアスコルビン酸塩の腸管前処置剤PLENVU(R)のハイクオリティー洗浄効能を紹介した。大腸内視鏡検査の現場評価で、ピコスルファートナトリウムにマグネシウム塩CITRAFLEET(R)、2リットルPEGにアスコルビン酸塩MOVIPREP(R)をそれぞれ加えたものと比較した(注1、2)。また、DAYB研究のデータは、Boston Bowel Preparation Scoreによる適正洗浄率でマグネシウム塩入りピコスルファートナトリウムと比べて、PLENVU(R)が統計上有意の改善を達成したことを示した(注3)。

高い洗浄効能:

*DAYB研究によると、前日投与のマグネシウム塩入りピコスルファートナトリウムCITRAFLEET(R)に比べて、PLENVU(R)は有意の洗浄成功率を達成

――結腸全体で73.3% vs 60.9%、P=0.003(注1)

――上行結腸で36.3% vs 15.4%、P<0.001(注1)

*MORA研究によると、標準のアスコルビン酸塩MOVIPREP(R)入り2リットルPEGに比べて、PLENVU(R)は有意の改善率を達成

――夜と午前に分割投与した場合、結腸全体の洗浄率は97.0% vs 90.9%、P=0.003(注2)

――夜間に分割投与した場合、上行結腸のハイクオリティー洗浄率は74.8% vs 60.9%、P<0.001、また、午前だけ投与した場合は75.8% vs 60.9%、P<0.001(注2)

夜だけ投与した場合の洗浄率改善(Boston Bowel Preparation Scale)をマグネシウム塩入りピコスルファートナトリウムと比較:

*PLENVU(R)はBoston Bowel Preparation Scaleで患者全体(61.9% vs 47.3%、P=0.001)、上行結腸(66.9% vs 50.2%、P<0.001)のいずれも統計上有意な適正水準上昇を達成(注3)

PLENVU(R)は研究で、標準のアスコルビン酸塩入り2リットルPEG、マグネシウム塩入りピコスルファートナトリウム、チオ硫酸塩ソリューションに比べて良好な耐容性を示した(注4、5、6)。

Norgineの最高開発・医療責任者であるアラステア・ベンボウ博士は「これら第3相のデータは標準の腸管前処置に代わる低容量の腸管前処置、PLENVU(R)の将来性を確認した。結腸内視鏡検査は最も効果がある大腸検査法の1つと見なされており、PLENVU(R)のような高効能の腸管前処置の利用は、よりよい腺腫やポリープの検知にとって重要であり、最終的には患者の転帰を改善し、医療制度のリソースを節約する」とコメントした。

これらのデータは2017年5月6-9日の米国消化器病週間(DDW、シカゴ)で発表された。

PLENVU(R) 第3相臨床試験プログラムは多施設、ランダム、平行グループ研究3件、NOCT、MORAおよびDAYBを含んでいる。

大腸がんは欧州、米国で2番目に多いがん関連の死因になっており、欧州では41万2000人、米国では13万6115人の大腸がん患者が毎年見つかっている(注7、8)。

PLENVU(R)データはDDW で5月6日(土)正午(米中部夏時間)に発表。

Norgineは2016年8月、Valeant Pharmaceuticalsと米国とカナダにおけるPLENVU(R)のライセンス契約を結んだ。

PLENVU(R)はまだ米国、欧州で使用承認されていない。Norgineは欧州では2017年、米国で2018年に規制当局の承認を得られると期待している。

Norgine Presents New Post-hoc Data Analyses Highlighting the Efficacy of PLENVU(R) for Bowel Cleansing at DDW

     Norgine B.V. today presented new post-hoc analyses from the phase III DAYB

and MORA studies that demonstrate the high-quality cleansing efficacy of

PLENVU(R), a low volume, 1 litre PEG and ascorbate bowel preparation when

compared to sodium picosulfate with magnesium citrate (CITRAFLEET(R)) and 2

litre PEG with ascorbate (MOVIPREP(R)) respectively when using site

colonoscopist assessments.[1],[2] In addition, data from the DAYB study show

that PLENVU(R) achieved statistically significant improvements in adequate

cleansing rates using the Boston Bowel Preparation Score when compared to

sodium picosulfate with magnesium citrate.[3]

High cleansing efficacy:

     * In the DAYB study, when compared to sodium picosulfate with magnesium

citrate (CITRAFLEET(R)) using a day before dosing, PLENVU(R) achieved

significantly better cleansing success;

          -- in the overall colon (73.3% vs 60.9%, P=0.003)[1]

          -- and a high-quality cleansing of the ascending colon (36.3% vs

15.4%, P<0.001)[1]

     * In the MORA study, when compared to standard 2 litre PEG with ascorbate

(MOVIPREP(R)), PLENVU(R) achieved a significantly improved rate of;

          -- cleansing in the overall colon when administered as an

evening/morning split-dose (97.0% vs 90.9%, P=0.003)[2]

          -- high-quality cleansing in the ascending colon when administered as

an overnight split-dose (74.8% vs 60.9%, P<0.001) or morning-only dose (75.8%

vs 60.9%, P<0.001)[2]

Improved cleansing (Boston Bowel Preparation Scale) in evening only dosing when

compared to sodium picosulfate with magnesium citrate;

     * PLENVU(R) achieved a statistically significantly increased proportion of

adequate level cleansing of the overall (61.9% vs 47.3%, P=0.001) and ascending

colon (66.9% vs 50.2%, P<0.001) in patients with Boston Bowel Preparation Scale

scores[3]

     PLENVU(R) has been shown to be well tolerated in studies versus standard 2

litre PEG with ascorbate, sodium picosulfate with magnesium citrate and

trisulfate solution.[4],[5],[6]

     The PLENVU(R) Phase III clinical trial programme includes three

multicentre, randomised, parallel group studies: NOCT, MORA, and DAYB.

     Colorectal cancer is the second most common cause of cancer-related

mortality in Europe and the US, with 412,000 new diagnoses of colorectal cancer

every year in Europe and 136,115 in the US.[7],[8]

     In August 2016, Norgine entered into a licensing agreement with Valeant

Pharmaceuticals for PLENVU(R) in the US and Canada.

     PLENVU(R) is not yet approved for use in the US or Europe. Norgine

anticipates regulatory approval in Europe in 2017 and in 2018 in the US.
