メルク(Merck)は2015年9月25-29日にオーストリアのウィーンで開催される今年の欧州がん学会(ECC)で、再度注目された腫瘍、がん免疫パイプラインの初期および後期化合物とともにアービタックス(R)に関する新しいデータを発表する。
これらデータは、新しい療法を開発する同社の科学依拠・患者中心のアプローチを強化するもので、膵がん、非小細胞肺がん、尿路上皮がんなど治療の難しいがんと闘う患者を助ける医薬品である。メルクセローノ(Merck Serono)のルチアーノ・ロセッティ氏(メルクバイオ医薬ビジネス部門グローバル研究・開発部長)は「ECC 2015で発表する当社データは、社外イノベーション、治療を個人に合わせるための適格治療(precision medicine)へのフォーカスを通じて、当社腫瘍学戦略が有効であることを示している。われわれは患者の転帰を向上する重要な要素である、患者が当社治療法から多くの恩恵を受けことを理解することにコミットしている」と語った。
▽患者のニーズに応えるコラボレーション
今年のECCでがんに挑戦するデータを発表する2つの最も戦略的コラボレーションは、メルク・ファイザー提携とThreshold Pharmaceuticals, Inc.とメルクの提携関係である。
メルクとファイザーの提携は、PD-L1阻害薬の潜在的役割と研究中のがん免疫剤avelumabの安全性と効能に関する6つの論文要約を発表する。この免疫チェックポイント阻害剤の新しいデータは、尿路上皮がん(例えば膀胱)、胸膜中皮腫、胃・食道胃接合部がんで発表される。IB相試験から得られた別の非小細胞肺がんと卵巣がんのデータは、米臨床腫瘍学会の2015年年次会議ですでに公表されたものに基づいて得られたものである。(1-10)
メルクはThreshold Pharmaceuticals Inc.と共同して、進行性固形がんと膵がんにかかったアジアの患者にevofosfamide(これまでTH-302として知られていた低酸素活性化プロドラッグ)を投与する第I相研究データを発表する。米食品医薬品局(FDA)は、局所で進行している切除不能もしくは転移性のがん治療薬のゲムシタビン(gemcitabine)と組み合わせるか、局所で進行している進行性もしくは転移性の軟部性組織肉腫にかかった患者治療用のドキソルビシン(doxorubicin)と併用処方されるevofosfamideに対して優先承認審査指定薬制度を適用した。
▽患者個人に合わせた治療
今年のECCで発表されるデータはまた、患者の個人に合わせた治療を重視するメルクを強調する。これはc-MET(肝細胞増殖因子の受容体であるチロシンキナーゼ)を過剰発現する進行性の肝細胞がんと非小細胞肺がんにかかった患者に対するc-MET受容体チロシンキナーゼの選択的低分子阻害剤であるtepotinibのデータが含まれる。さらに、第1選択薬RAS遺伝子野生型の転移性結腸直腸がん(mCRC)の治療用治験薬アービタックスおよび頭頸部がんのバイオマーカーの臨床的価値に関する新たな部分解析も発表される。

▽アービタックス(Erbitux)について
アービタックスは上皮成長因子受容体(EGFR)を標的とするIgG1モノクローナル抗体である。アービタックスの作用機序は、モノクローナル抗体として、EGFRに対して特異的に結合するという点で従来の標準的な化学療法とは明確に異なる。EGFRに結合することによって受容体の活性化と下流のシグナル伝達が阻害され、正常組織への腫瘍細胞の増殖、浸潤と転移が抑制される。また、化学療法や放射線療法によって引き起こされた損傷を修復する腫瘍細胞の活性を抑制し、腫瘍内血管新生を抑制するとも考えられており、それによって腫瘍の増殖を全体的に抑えるとされてる。
アービタックスに最も多い副作用はざ瘡様皮疹であり、皮疹の程度(グレード)と治療効果は相関があると報告されている。また、結腸・直腸がんの患者を対象とした使用成績調査において、約5%に過敏反応(インフュージョン・リアクション)が発生し、このうちの約2分の1が重度な症状を示した。
アービタックスは結腸・直腸がんおよび頭頸部扁平上皮がんの治療薬として、90カ国以上で市販承認を受けている。
メルクは1998年、イーライリリーの100%子会社であるイムクロン社から米国、カナダ以外でのアービタックスの販売権をライセンス供与された。メルクはグローバルでがん治療の進展に継続的にコミットしており、対象領域における新規治療法の研究に取り組んでいる。
▽evofosfamideについて
evofosfamide(TH-302)は臨床試験中の薬剤で低酸素活性化プロドラッグであり、多くの固形がんで顕著に認められる重度の低酸素状態下で活性化すると考えられてる。固形がんの内部では血管形成が不十分なため、低酸素状態となっている。同様に、造血器腫瘍患者の骨髄においても高度の低酸素状態を認める場合がある。
evofosfamideは現在、次の2つの第III相試験において評価中である。1つは、局所進行切除不能または転移性軟部肉腫(STS)患者を対象にドキソルビシンとの併用投与とドキソルビシン単剤投与を比較する試験(TH-CR-406試験)であり、もう1つは、局所進行切除不能または転移性の膵がん患者を対象にevofosfamide+ゲムシタビンとプラセボ+ゲムシタビンを比較する試験(MAESTRO試験)である。いずれの第III相試験も、FDAとの臨床試験計画評価(Special Protocol Assessment(SPA))合意に基づいて実施されてる。FDAと欧州委員会(European Commision)は、evofosfamideをSTSおよび膵がん治療のオーファンドラッグに指定した。evofosfamideについては、非扁平上皮・非小細胞肺がんの治療に関する第II相試験ならびにその他の固形がんと造血器腫瘍を対象とした初期の臨床試験が進行中である。
2012年2月、メルクはThreshold Pharmaceuticals, Inc.と、evofosfamideの米国での共同販売オプションを有するグローバルな共同開発および販売に関する契約を締結した。
▽tepotinibについて
tepotinib(もしくはMSC2156119J)は臨床試験中の薬剤でc-Met受容体チロシンキナーゼの低分子阻害剤であり、前臨床試験において肝細胞増殖因子依存性と非依存性腫瘍の増殖を阻害し退縮させることが示された。C-Metシグナル伝達経路の変更はさまざまなタイプのがんで見られ、腫瘍の増殖・悪性化や臨床予後の不良と相関している。tepotinibは現在、第I/II相試験の評価中である。
▽avelumabについて
avelumab(もしくはMSB0010718C)は、臨床試験中の薬剤で完全ヒト型抗PD-L1 IgG1モノクローナル抗体(fully human anti-PD-L1 IgG1 monoclonal antibody)である。avelumabはPD-L1の作用を抑制することにより、T細胞と適応免疫系を活性化すると考えられている。Fc領域を維持することにより、avelumabは自然免疫系に働きかけ、抗体依存性細胞媒介性細胞傷害(ADCC)を誘発すると考えられている。2014年11月、メルクとファイザーはavelumab の共同開発および共同販売をする戦略的提携の締結を発表した。

Collaborating to Address Patient Needs
Two important strategic collaborations presenting data in challenging
cancers at ECC this year are the Merck-Pfizer Alliance, and Merck's partnership
with Threshold Pharmaceuticals, Inc.
The Merck-Pfizer Alliance will present six abstracts on studies evaluating
the potential role of PD-L1 inhibition, and the safety and efficacy of the
investigational cancer immunotherapy avelumab. New data on this immune
checkpoint inhibitor will be presented in urothelial (e.g. bladder),
mesothelioma and gastric/gastroesophageal cancers. Additional non-small cell
lung and ovarian cancer data from Phase Ib trials build on those previously
presented at the 2015 Annual Meeting of the American Society of Clinical
Oncology.[1-10]
Together with Threshold Pharmaceuticals Inc., Merck will present Phase I
study data on evofosfamide (previously known as TH-302; an investigational
hypoxia-activated prodrug) in Asian patients with advanced solid tumors and
pancreatic cancer. The U.S. Food and Drug Administration (FDA) granted Fast
Track designation to evofosfamide, administered in combination with gemcitabine
for the treatment of previously untreated patients with locally advanced
unresectable or metastatic pancreatic cancer, and for the development of

locally advanced or metastatic soft tissue sarcomas.
Personalizing Treatment for Patients
Data presented at ECC this year will also highlight Merck's focus on
precision medicine with the aim to deliver personalized treatments for
patients. This includes data for tepotinib, a selective small-molecule
inhibitor of the c-Met receptor tyrosine kinase, in patients with advanced
hepatocellular and non-small cell lung cancer that overexpress c-Met. In
addition, new subanalyses on the clinical value of Erbitux in the treatment of
1st line RAS wild-type metastatic colorectal cancer (mCRC), and of biomarkers
in head and neck cancer will also be presented.
Cutting-Edge Diagnostics to Ensure Appropriate Treatment
Merck's precision medicine research extends beyond treatment into
collaborations to aid in the development of cutting-edge diagnostic tests that
help physicians rapidly identify patients that will most likely benefit from
specific treatments. Merck is collaborating with Sysmex Inostics on the
development and commercialization of a liquid biopsy test for determining the
RAS (KRAS and NRAS) tumor mutation status in mCRC patients. Sysmex will present
data on the validity of the test, also referred to as a blood-based biomarker
test, at ECC. The liquid biopsy RAS biomarker test is expected to receive its
European Conformity approval (CE mark) this year. Currently it is applied for
research use only (RUO) in a number of countries.

About Erbitux
Erbitux(R) is a highly active IgG1 monoclonal antibody targeting the
epidermal growth factor receptor (EGFR). As a monoclonal antibody, the mode of
action of Erbitux is distinct from standard non-selective chemotherapy
treatments in that it specifically targets and binds to the EGFR. This binding
inhibits the activation of the receptor and the subsequent signal-transduction
pathway, which results in reducing both the invasion of normal tissues by tumor
cells and the spread of tumors to new sites. It is also believed to inhibit the
ability of tumor cells to repair the damage caused by chemotherapy and
radiotherapy and to inhibit the formation of new blood vessels inside tumors,
which appears to lead to an overall suppression of tumor growth.
The most commonly reported side effect with Erbitux is an acne-like skin
rash that seems to be correlated with a good response to therapy. In
approximately 5% of patients, hypersensitivity reactions may occur during
treatment with Erbitux; about half of these reactions are severe.
Erbitux has already obtained market authorization in over 90 countries
world-wide for the treatment of colorectal cancer and for the treatment of
squamous cell carcinoma of the head and neck (SCCHN).
Merck licensed the right to market Erbitux outside the U.S. and Canada from
ImClone LLC, a wholly-owned subsidiary of Eli Lilly and Company, in 1998. Merck
has an ongoing commitment to the advancement of oncology treatment and is
currently investigating novel therapies in highly targeted areas.
About Evofosfamide
Evofosfamide (previously known as TH-302) is an investigational
hypoxia-activated prodrug that is thought to be activated under severe tumor
hypoxic conditions, a feature of many solid tumors. Areas of low oxygen levels
(hypoxia) in solid tumors are due to insufficient blood vessel supply.
Similarly, the bone marrow of patients with hematological malignancies has also
been shown, in some cases, to be severely hypoxic.
Evofosfamide is currently under evaluation in two Phase III trials: one in
combination with doxorubicin versus doxorubicin alone in patients with locally
advanced unresectable or metastatic soft tissue sarcoma (the TH-CR-406 trial),
and the other in combination with gemcitabine versus gemcitabine and placebo in
patients with locally advanced unresectable or metastatic pancreatic cancer
(the MAESTRO trial). Both Phase III trials are being conducted under Special
Protocol Assessment (SPA) agreements with the FDA. The FDA and the European
Commission have granted evofosfamide Orphan Drug designation for the treatment
of STS and pancreatic cancer. The FDA has also granted Fast Track designation
to evofosfamide for the treatment of pancreatic cancer and soft tissue sarcoma.
Evofosfamide is also being investigated in a Phase II trial for the treatment
of non-squamous non-small cell lung cancer, and in earlier-stage clinical
trials of other solid tumors and hematological malignancies.
Merck signed a global license and co-development agreement for evofosfamide
with Threshold Pharmaceuticals, Inc. in February 2012, with an option for
Threshold to co-commercialize in the U.S.
About Tepotinib
Tepotinib (also known as MSC2156119J or EMD1214063) is an investigational
small-molecule inhibitor of the c-Met receptor tyrosine kinase that has been
shown to cause growth inhibition and regression of tumors with c-Met mutations,
c-Met overexpression or hepatocyte growth factor secretion in preclinical
models. Tepotinib is currently under evaluation in Phase I/II trials.
About Avelumab
Avelumab (also known as MSB0010718C) is an investigational fully human
anti-PD-L1 IgG1 monoclonal antibody. By inhibiting PD-L1 interactions, avelumab
is thought to enable the activation of T-cells and the adaptive immune system.
By retaining a native Fc-region, avelumab is thought to engage the innate
immune system and induce antibody-dependent cell-mediated cytotoxicity (ADCC).
In November 2014, Merck and Pfizer announced a strategic alliance to co-develop
and co-commercialize avelumab.
